Sometimes new experiences can be confusing and stressful. Our goal at ADIO Chiropractic here in Mission Viejo is to make you feel welcome, provide a thorough evaluation, answer your questions, and feel completely comfortable on your first visit.

Health Intake Forms

When you first arrive at ADIO Chiropractic you will be greeted and given our new practice member paperwork. This is our opportunity to get to know you! Please fill out all paperwork as completely and thoroughly as possible and sign all forms. Paperwork can be printed and filled out before your appointment, this will save you about 15-20 minutes on your first day in our office.
